Some state‐based newborn screening programs in the United States already use sequencing technology, as a secondary screening test for individual conditions rather than as a broad screening tool. Newborn screening programs sequence an individual gene, such as the cystic fibrosis transmembrane conductance regulator, which causes cystic fibrosis, after an initial biochemical test suggests that a baby might have a condition related to that gene. The experiences of state public health departments with individual‐gene sequencing illustrate both the usefulness of the technology (...) and its complexities. Here I discuss how newborn screening programs investigate cystic fibrosis and, as another example, adrenoleukodystrophy through individual gene sequencing. (shrink)

Taking Wittgenstein at His Word is an experiment in reading organized around a central question: What kind of interpretation of Wittgenstein's later philosophy emerges if we adhere strictly to his claims that he is not in the business of presenting and defending philosophical theses and that his only aim is to expose persistent conceptual misunderstandings that lead to deep philosophical perplexities? Robert Fogelin draws out the therapeutic aspects of Wittgenstein's later work by closely examining his account of rule-following and (...) how he applies the idea in the philosophy of mathematics.The first of the book's two parts focuses on rule-following, Wittgenstein's "paradox of interpretation," and his naturalistic response to this paradox, all of which are persistent and crucial features of his later philosophy. Fogelin offers a corrective to the frequent misunderstanding that the paradox of interpretation is a paradox about meaning, and he emphasizes the importance of Wittgenstein's often undervalued appeals to natural responses. The second half of the book examines how Wittgenstein applies his reflections on rule-following to the status of mathematical propositions, proofs, and objects, leading to remarkable, demystifying results.Taking Wittgenstein at His Word shows that what Wittgenstein claims to be doing and what he actually does are much closer than is often recognized. Arguing that criticisms have--from the very start--rested on misreadings, Fogelin begins by providing a narrative of the way Hume’s argument actually unfolds. What Hume’s critics (and even some of his defenders) have failed to see is that Hume’s primary argument depends on fixing the appropriate standards of evaluating testimony presented on behalf of a miracle. Given the definition of a miracle, Hume quite reasonably argues that the standards for evaluating such testimony must be extremely high. All international codes of research ethics and virtually all national legislation and regulation in the field of research involving human subjects project an attitude of protectionism. Written with the aim of avoiding a repetition of atrocities like those committed by the Nazi physician-researchers, calamities like the thalidomide experience, or ethical violations like those of the Tuskegee syphilis study, their dominant concerns are the protection of individuals from injury and from exploitation. In recent years, however, society's perception of clinical research has (...) shifted dramatically. Now, largely as a consequence of the efforts of the AIDS activists, clinical research is widely perceived as benign and beneficial. Although this shift in attitude has resulted in some important improvements in research policies and practices, this new perception is just as wrong-headed as was the earlier excessive protectionism. It is necessary to maintain a balanced perspective; our policies should encourage the conduct of ethical research while maintaining the vigilance necessary to safeguard the rights and welfare of the subjects. Several scholars and many religiously conservative thinkers have recently charged that Hitler’s ideas about race and racial struggle derived from the theories of Charles Darwin (1809-1882), either directly or through intermediate sources. So, for example, the historian Richard Weikart, in his book From Darwin to Hitler , maintains: “No matter how crooked the road was from Darwin to Hitler, clearly Darwinism and eugenics smoothed the path for Nazi ideology, especially for the Nazi stress on expansion, war, racial struggle, and racial (...) extermination.” In a subsequent book, Hitler’s Ethic: The Nazi Pursuit of Evolutionary Progress, Weikart argues that Darwin’s “evolutionary ethics drove him [Hitler] to engage in behavior that the rest of us consider abominable.” Other critics have also attempted to forge a strong link between Darwin’s theory and Hitler’s biological notions. In the 2008 film “Expelled,” a documentary defense of Intelligent Design, the Princeton trained philosopher David Berlinski, in conversation with Weikart, confidently asserts: “If you open Mein Kampf and read it, especially if you can read it in German, the correspondence between Darwinian ideas and Nazi ideas just leaps from the page.” John Gray, former professor at the London School of Economics, does allow that Hitler’s Darwinism was “vulgar.” Hannah Arendt also appears to have endorsed the connection when she declared: “Underlying the Nazis’ belief in race laws as the expression of the law of nature in man, is Darwin’s idea of man as the product of a natural development which does not necessarily stop with the present species of human being.” Put “Darwin” and “Hitler” in a search engine and several million hits will be returned, most from religiously and politically conservative websites, articles, and books. Next SectionIn a series of previous articles, we have considered how we might reconceptualise central themes in medicine and medical education through ‘thinking with Homer’. This has involved using textual approaches, scenes and characters from the Iliad and Odyssey for rethinking what is a ‘communication skill’, and what do we mean by ‘empathy’ in medical practice; in what sense is medical practice formulaic, like a Homeric ‘song’; and what is lyrical about medical practice. Our approach is not to historicise medicine (...) and medical education, but to use thinking with Homer as a medium and metaphor for questioning the habitual and the taken-for-granted in contemporary practice. In this article, we tackle the complex theme of ‘translation’. We use the lens of translation studies to examine the process of turning the patient's story into medical language. We address the questions: what makes a ‘good’ translation? What are the consequences of mistranslation and poor translation? And, while things are inevitably lost in translation, does this matter? What Kant in fact proposes is that we judge a work to be the product of genius as a consequence of our judgment of its beauty. As Kant explains in KU §58, when we judge the beautiful in fine art it is the (...) indeterminacy of the mode of mental activity in which aesthetic ideas arise which permits us to interpret idealistically the purposiveness in the work, on which account we regard it as the product of genius. This article examines in particular KU §50 concerning the combination of genius and taste, and the puzzling assertion in KU §51 that all beauty is the expression of aesthetic ideas. (shrink)

Classicists and connectionists alike claim to be able to explain systematicity. The proposed classicist explanation, I argue, is little more than a promissory note, one that classicists have no idea how to redeem. Smolensky's (1995) proposed connectionist explanation fares little better: it is not vulnerable to recent classicist objections, but it nonetheless fails, particularly if one requires, as some classicists do, that explanations of systematicity take the form of a‘functional analysis’. Nonetheless, there are, I argue, reasons for cautious optimism about (...) the prospects of a connectionist explanation. (shrink)

‘The Natural System’ is the abstract notion of the order in living diversity. The richness and complexity of this notion is revealed by the diversity of representations of the Natural System drawn by ornithologists in the Nineteenth Century. These representations varied in overall form from stars, to circles, to maps, to evolutionary trees and cross-sections through trees. They differed in their depiction of affinity, analogy, continuity, directionality, symmetry, reticulation and branching, evolution, and morphological convergence and divergence. Some representations were two-dimensional, (...) and some were three-dimensional; n-dimensional representations were discussed but never illustrated. The study of diagrammatic representations of the Natural System is made difficult by the frequent failure of authors to discuss them in their texts, and by the consequent problem of distinguishing features which carried meaning from arbitrary features and printing conventions which did not. Many of the systematics controversies of the last thirty years have their roots in the conceptual problems which surrounded the Natural System in the late 1800s, problems which were left unresolved when interest in higher-level systematics declined at the turn of this century. Accounts of the evolutionary past have as much in common with works of narrative history as they do with works of science. Awareness of the narrative character of evolutionary writing leads to the discovery of a host of fascinating and hitherto unrecognized problems in the representation of evolutionary history, problems associated with the writing of narrative. These problems include selective attention, narrative perspective, foregrounding and backgrounding, differential resolution, and the establishment of a canon of important events. The narrative aspects of (...) evolutionary writing, however, which promote linearity and cohesiveness in conventional stories, conflict with the underlying chronicle of evolution, which is not linear, but branched, and which does not cohere, but diverges. The impulse to narrate is so great, however, and is so strongly reinforced by traditional schemes of taxonomic attention, that natural historians have more often abandoned the diverging tree than they have abandoned the narrative mode of representation. If we are to understand the true nature of the evolutionary past then we must adopt “tree thinking”, and develop new and creative ways, both narrative and non-narrative, of telling the history of life. The sciences of ethology and sociobiology have as premisses that certain dispositions and behavioural patterns have evolved with species and, therefore, that the acts of individual animals and men must be viewed in light of innate determinates. These ideas are much older than the now burgeoning disciplines of the biology of behaviour. Their elements were fused in the early constructions of evolutionary theory, and they became integral parts of the developing conception. Our image of Herbert Spencer is that of a bald, dyspeptic bachelor, spending his days in rooming houses, and fussing about government interference with individual liberties. Beatrice Webb, who knew him as a girl and young woman recalls for us just this picture. In her diary for January 4, 1885, she writes: Royal Academy private view with Herbert Spencer. His criticisms on art dreary, all bound down by the “possible” if not probable. That poor old man would miss me on (...) the whole more than any other mortal. Has real anxiety for my welfare—physical and mental. Told him story of my stopping cart horse in Hyde Park and policeman refusing to come off his beat to hold it. Want of public spirit in passers-by not stopping it before. “Yes, that is another instance of my first principle of government. Directly you get state intervention you cease to have public spirit in individuals; that will be a constantly increasing tendency and the State, like the policeman, will be so bound by red-tape rules that it will frequently leave undone the simplest duties.”1 Spencer appears a man whose strangled emotions would yet cling to a woman whose philosophy would be completely alien to his own, as Webb’s Fabian Socialism turned out to be. Our image of Darwin is more complex than our image of Spencer. We might think of him nestled in the bosom of his large family, kindly, and just a little sad. The photo of him taken by Julia Cameron reveals the visage of an Old Testament prophet, though one, not fearsome, but made wise by contemplating the struggle of life on this earth. These images have deeply colored our reaction to the ideas of each thinker. The pictures are not false, but they are cropped portraits that tend to distort our reactions to the theories of each.
